Ext: Stylish, icons and adjustments

I get 2 already exist styles for Stylish manager/editor, mix them and add/remove some features.

What this style do?

Menu:
Added icons for every position in menu.

Manager:
1. All buttons flat and have they own icons. Clear Filter button flat too, but don't "rise" when hovered.
2. Removed all useless margins and padding around controls, all controls slim and don't eat more space than they need.
3. Added ribbon-style to list of styles.

Editor:
1. Removed all useless margins and padding around controls, all controls slim and don't eat more space than they need.
2. All buttons at the top flat, have they own icons and don't "rise" when hovered.
3. Fixed position of scrollbars.

Style compatible with Stylish 0.5.2 and Fx3. Also this compilation don't make dramatic damage to other forms, because removed ugly style for checkboxes, all checkboxes (except in Stylish menu) have they default style.
